In order for your son to participate in this ministry, we must have all of the following completed...

    If your son has been on a youth event with Crabapple First Baptist Church in the last 12 months, then you already have completed this form for your son. If not, then you'll need to complete this form and have it notarized. There are notaries at the church on Sunday mornings and evenings and Wednesday evenings.

    We want to make sure you understand what's involved in this ministry, and so we will ask the parent(s) or guardians of each boy to print and sign this Parental Consent Form.

    If you as a parent would like to know in advance what the upcoming activity or activities are, you can request a password for this website that will allow you to see the specific adventures that are coming up, so you can be prepared for them, and so you can help your son decide what clothes and shoes to wear.

    Our only request is that if you have a password that you keep it to yourself and keep the adventures hush-hush. Sometimes the success or failure of an event depends on the "surprise factor". If some of the boys already know what's coming, the impact may not be as effective, and the adventure may not be fair to all the boys.
